This is a Maintenance and/or Support Agreement between Donaldson
Consulting and QISV Customer to maintain and support an existing
and/or new computer network and/or equipment.
This agreement includes 10 hours of maintenance services for a
flat fee of $500 a month. Any hours exceeding the 10 hours will
be billed at a rate of $67.50 per hour. The annual contract agreement
will begin on the date signed by both parties and will end exactly
one year from the starting date. Payments can be made monthly
by the 5th of each month, semiannually beginning on the start
date and then on the 5th day of the 6th month, or annually due
on the start date. All pricing reflects List and State discounts.
